Summer Associate - PHMC Health Network VISTA

Member Duties : 1. Provide comprehensive guidance to patients and community members on how to access community resources. 2. Assist patients in applying to Medicaid and SNAP benefits. 3. Work collaboratively with patients to access integrative healthcare services, including routine primary care, mental health, and substance use services, as needed. 4. Support ongoing initiatives to conduct outreach and prevention efforts for infectious diseases in the community.

Service Description : PHMC Health Network is comprised of five Federally Qualified Health Centers (FQHCs) providing affordable, accessible, primary medical and behavioral health care and preventive services to the uninsured and underinsured patients in its service area. Access to health insurance is crucial for Health Network patients, as it helps ensure they have access to all the medications and specialty care they need to manage their complex health care needs. The PHMC Health Network is therefore seeking AmeriCorps VISTA Summer Associates to support patients and community members in maintaining access and reconnecting to public benefits and accessing preventive health screenings to achieve and maintain essential health care access.
