JOB REQUIREMENTS
As a Case Manager RN, you’ll work closely with the interdisciplinary team to support safe, efficient, and patient-centered care:
- Conduct comprehensive patient assessments, including psychosocial, financial, and support system needs
- Develop and coordinate individualized care and discharge plans
- Collaborate with providers, care teams, and external partners to ensure smooth transitions of care
- Identify and remove barriers to discharge and patient progression
- Facilitate transfers to appropriate levels of care or post-acute services
- Support patients and families with education, counseling, and care planning
- Monitor length of stay and resource utilization to support quality and efficiency
- Advocate for patients while coordinating with payers and post-acute providers
- Provide clear, timely documentation and communication across care teams
- Support complex cases including behavioral health, abuse concerns, and end-of-life planning
This role blends clinical expertise with coordination, communication, and critical thinking.
JOB DUTIES
Education
- Graduate of an accredited Nursing program
Licensure & Certifications
- Current Michigan RN license required
- BLS preferred
- Case Management certification preferred
Experience
- Minimum 3 years of RN experience in an acute care setting
- Experience in case management, discharge planning, or utilization management preferred
Skills
- Strong communication and collaboration skills
- Ability to manage complex patient needs and navigate care transitions
- Critical thinking and problem-solving in a fast-paced environment
- Ability to work independently and exercise sound judgment
- Experience working with interdisciplinary teams and external partners
- Comfortable with documentation and EMR systems
